More About This Website

CHWs are community members who help other members of their communities get healthier. They act as liaisons between community members and health-care and social services systems; provide culturally-responsive education about health issues; extend the provision of health-care services; help community members prevent and manage chronic diseases; create opportunities for community members to come together to address health issues; and model healthful behaviors. Very busy, I'd say!
